The company updates a service policy, but its internal AI assistant continues quoting the previous rule. Changing the source document does not by itself prove that every copy used for search and answer generation has been refreshed.

Before changing the model or rewriting the prompt, trace where the cited information came from. A business assistant may retrieve information from a search index, extracted text or another prepared knowledge store rather than opening the latest source file for every question.

Ask for an update path and a retirement path

Identify the authoritative document, its owner and the system that prepares it for retrieval. Record how a new version reaches that system, how failures are reported and what happens when a document is retired rather than revised.

Microsoft’s Azure Storage indexer guidance makes an important distinction: detecting changed content does not automatically provide deletion detection. A suitable deletion strategy is needed to avoid orphan search documents. This is one implementation example; other retrieval systems need their own documented update and removal mechanisms.

ITZ’s AI integration and business knowledge assistant service can help map that knowledge lifecycle. Request an AI knowledge refresh review with the source types, content owners and intended refresh requirement. Use redacted examples in the enquiry.

Define freshness as a business requirement

“Keep the assistant up to date” is too vague for acceptance. Decide which changes require urgent withdrawal from answers, which can follow a normal refresh cycle and who verifies the result. A withdrawn procedure may require a different response from a routine formatting correction.

For a proposed service handbook, record the approved version, effective date and superseded version. The content owner should decide which material remains useful history and which must no longer support current advice. Keeping an archive and allowing it to answer current-policy questions are separate choices.

Run a controlled replacement test

  1. Create a harmless test document with a distinctive sample rule and an approved test question.
  2. Confirm the assistant retrieves it and that the citation identifies the expected source.
  3. Replace the sample rule through the supported publishing process. Record the source-change time and the next successful ingestion evidence.
  4. Ask the same question in a new conversation, then inspect the retrieved evidence as well as the answer.
  5. Retire a separate sample document and verify that it no longer appears as an active source for the test question.

Microsoft’s indexer execution guidance distinguishes scheduled and on-demand processing and explains how change detection drives incremental indexing. A job being triggered is not the same evidence as the required document appearing correctly in retrieval.

Check more than one place for stale material

If the old answer remains, investigate the retrieved document identity, duplicate copies, extraction output and any application cache. Check whether a previous conversation already contained the old text. Do not conclude that a search-index repair will remove information already included in a conversation.

Document the intervention the technical owner is allowed to make and how the business will verify recovery. Rebuilding a knowledge store can affect availability and cost; it should be a planned technical action, not the default response to every wrong answer.

Keep permissions and action controls intact

A content refresh must preserve the assistant’s document-access boundaries. Newer information is not automatically authorised for every employee. If documents can influence actions, retain the prompt-injection and action-boundary controls as well.

When the assistant cannot establish the current approved source, define a useful escalation: identify the uncertainty and direct the user to the responsible team. Do not promise that a refresh process eliminates every inaccurate answer. Retest representative questions after changes and retain source evidence for review.
